LAST MINUTE BOOKING FORM

This form is for any last minute bookings that are 72 hours or less and

require a $30.00 booking fee. 

**24 hours or less please use the special events form**

Hourly rates per sitter:

(rates very based off number of children)

1-2 children $30.00

3-4 children $35.00

4+ children $40.00

(there is a 3 hour minimum on all bookings)
